Building everyday skills for greater independence and stability

Commonwealth Community Development Services (CCDS) is launching Mental Health Skill-Building Services to support individuals who need help building the essential skills to live independently while managing the impact of mental health challenges. This intensive, home- and community-based service is designed for people who experience long-term mental health conditions, cognitive limitations, or dual diagnoses and would benefit from structured, one-on-one support.

Our qualified team uses a guided curriculum that addresses real-world needs such as securing housing, navigating transportation, managing healthcare routines, and preparing for employment. We also support daily living tasks like budgeting, maintaining a household, personal care, and emotional development. For clients with parenting responsibilities, the program includes skill-building to support their role as caregivers.

What Services May Be Included

Each individual receives a personalized service plan based on their goals and level of functioning. Services may involve assessing current skills, identifying support needs, and delivering regular, hands-on guidance in the home or community. We help individuals connect with local resources, advocate across healthcare and social service systems, and provide targeted education to build confidence in managing everyday tasks.

Our staff collaborates with other professionals involved in the individual’s care to ensure consistent communication and a unified approach to support.

Who Can Benefit

This program is designed for individuals with a mental health diagnosis who are working toward greater independence but need consistent support in daily living. It is especially helpful for those who are motivated to participate in ongoing, skills-focused sessions and are open to working collaboratively with a dedicated support provider.
